Are Your Kindergartners Ready?

It's an exciting time of year and lots to do before the first official day of school.

All Kindergarten physicals and immunizations record should be turned in to the health office by August 16.  All Kindergarten students who are planning to attend Oak Ridge Elementary School and Pine Meadow Elementary School in the fall of 2010-2011 (according to Minnesota School Immunization Law) must have a Kindergarten physical and updated immunization record on file with the health office prior to the first day of school in order to attend. 

The school health offices will be closed until August 30. You may still drop off forms in the main office during their hours of operation.

In addition to the standard immunization schedule, written proof that your child
has received the Hepatitis B three shot series, two MMR’s (Measles, Mumps, Rubella) and has had two varicella immunization OR has had the chicken pox
disease (your physician must verify the year with their signature).

Parents, if they so choose, may file a medical exception signed by a healthcare provider or conscientious objection signed by a parent/guardian and notarized.
